Introduction

In an effort to improve health services for the community, especially those with poor status, the Medan City Government launched the Medan Healthy Health Maintenance Guarantee Program (JPK-MS). This research focuses on implementing the JPK-MS program at the Matsum City Health Center, Medan Area District. The purpose of this study is to explore the program implementation process and identify the various obstacles encountered.

The JPK-MS program is designed to provide access to free health services for 500 thousand poor people in the city of Medan. The target of this program includes street vendors, rickshaw pullers, and poor teachers who are not registered in the Public Health Insurance Program (Jamkesmas). Unfortunately, there are still around 20% of poor people who have not yet received adequate health services, caused by data inaccuracy.

Background of the Study

The Medan City Government has been implementing various health programs to improve the health status of the community. However, despite these efforts, there are still many poor people who do not have access to health services. The JPK-MS program is designed to address this issue by providing free health services to the poor. However, the implementation of this program has been facing several challenges.

To achieve research goals, the author uses primary and secondary data. Data collection techniques are carried out through interviews, questionnaire distribution, direct observations, and documentation studies. Data analysis is carried out with a qualitative descriptive method that allows a deeper understanding of the problems encountered.

Obstacles in Program Implementation

Although the purpose of this program is noble, the implementation of JPK-MS at the Matsum City Health Center is not going well. There are several factors that result in this, including:

1. Delay in Implementation

This program is experiencing delays in implementation, which has a negative impact on the access of the poor on health services. The delay in implementation has resulted in a lack of awareness among the target population about the program, which has further exacerbated the problem.

2. Intensive Socialization

People who are entitled to benefit from this program do not get adequate information, so many do not know about JPK-MS. This lack of awareness has resulted in a low uptake of the program, which has further worsened the health status of the poor.

3. Weak Coordination and Communication

Lack of coordination between related parties makes the implementation process hampered. Many implementors do not understand the criteria of poor families, thus creating gaps in receiving benefits. This lack of coordination has resulted in a lack of trust among the target population, which has further exacerbated the problem.

4. Lack of Commitment

Low commitment from implementors and their awareness of the responsibility in running the JPK-MS program also contributes to the failure of this program. This lack of commitment has resulted in a lack of enthusiasm among the implementors, which has further worsened the implementation process.

Conclusion and Recommendations

Seeing from various obstacles, there needs to be an effort to improve the quality of the implementation of the JPK-MS program at the Matsum City Health Center. The government needs to ensure better socialization so that people know their rights for free health services. In addition, the improvement of the data of the poor becomes very important so that this program can be on target.

It is also important to improve communication and coordination between related parties, as well as strengthen the commitment and understanding of the implementors of this program. With the right steps, it is hoped that the JPK-MS program can have a positive impact on the poor in the city of Medan and guarantee better health access for them.

Recommendations for Future Research

Future research should focus on evaluating the effectiveness of the JPK-MS program in improving the health status of the poor. Additionally, research should be conducted to identify the factors that contribute to the success or failure of the program. This will help to inform policy and program development to improve the health status of the poor.

Limitations of the Study

This study has several limitations. Firstly, the study was conducted at a single site, which may not be representative of other sites.
